A Medication Review by a credentialled pharmacist

Our medication reviews provide consumer-centred, evidence-based recommendations to the resident's GP to enhance the safe and quality use of medicines.

A Geriatrician review

Our experienced geriatricians specialise in identifying ways to improve the health of older people, ensuring that care strategies align with their health goals and quality of life is maximised. Our nurse practitioners are also available to assist in care planning.

If the referral reason is for a dementia diagnosis or for a capacity assessment, please advise our team as this is not something that can be conducted via telehealth and will require an alternative service provider.

Transitional care support (e.g. new admission, pre/post hospital admission, respite care etc.)

We reduce health risks for residents, supporting them throughout their care journey while providing clear communication to themselves, their families and the whole care team. We prioritise enhancing communication at care transitions and ensure current (or created) care plans are clearly explained, available at handover while maintaining access to our assistance should anything unexpected happen.

Nurse practitioner collaboration

Our experienced nurse practitioners provide adaptable services to care for your resident(s) in a variety of ways, including (but not limited to) wound and pain management, sleep assistance and palliative support. They work independently but can also collaborate closely with GP’s and other care providers to ensure your resident’s needs are met.
